  • Go to https://carlopacific.com/vip/maxicare. Select the type of Maxicare Prepaid Health Card listing you would like to purchase and click ‘Add to Cart’. When you’re done shopping, click ‘View Cart’ to make sure you have accurate information. ‘Proceed to checkout’. You will be receiving a confirmation email with activation instructions on your Maxicare Prepaid Health Card.

Important Reminders

–  The PRIMA Silver card must be presented at all times, alongside one (1) valid government – issued ID (i.e. Driver’s License/Passport)
–  Membership is valid up to one(1) year from the date of registration
–  These cards are non-transferable once registered.
–  Voucher must be registered within three (3) months from the date of release
–  Payment slip from any of our partner banks is only valid for three (3) months from the date of payment
–  No refund policy
