When you’re done shopping, click ‘View Cart’ to make sure you have accurate information. ‘Proceed to checkout’. You will be receiving a confirmation email with activation instructions on your Maxicare Prepaid Health Card.

Important Reminders

–  The PRIMA Gold card must be presented at all times, alongside one (1) valid government – issued ID (i.e. Driver’s License/Passport/Senior Citizen Card)
–  Membership is valid up to one(1) year from the date of registration
–  These cards are non-transferable once registered.
–  Voucher must be registered within three (3) months from the date of release
–  Payment slip from any of our partner banks is only valid for three (3) months from the date of payment
